This early intervention platform is built by Wayfinder, a public benefit company dedicated to connecting Colorado’s disability community for better health. Wayfinder also develops referral management and resource navigation software for Home and Community-Based Services (HCBS) and Long-Term Services and Supports (LTSS) providers.
How do providers receive referrals?
Licensed early intervention providers can claim a free profile, browse open referrals, and respond to opportunities that match their expertise. They can also opt in to receive email notifications for new referrals aligned with their profile.
